First Choice Health Network Preferred Provider Organization does business with providers who have Practice Locations in the following states:

Alaska, Alabama, Arizona, California, Colorado, Georgia, Idaho, Iowa, Kansas, Kentucky, Minnesota, Mississippi, Montana, Nebraska, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Tennessee, Texas, Utah, Washington, Wisconsin and Wyoming

1. Verify that we accept applications for your provider specialty. Check here to verify.
2. We require all practitioners that we credential to have a current CAQH application. If you don’t have a currently attested CAQH application, please register to start your CAQH application here.
3. Once you have completed the CAQH Application, make sure you allow global access and that all supporting documents are current as well as your attestation date. After this is done, you can proceed to the next step.
4. You will need to fill out the FCH Provider Interest Form, including your individual NPI number and CAQH number to complete the form.
5. Once we receive your completed form, we will begin the credentialing process. Please allow between 60 - 90 days for the processing of your application.
   
   (NOTE: Our credentialing vendor is CertifyOS and they may reach out for any missing information. Please check your spam folder regularly in case these emails go to spam.)

We do not accept paper applications.

If you are a DME, Lab, Hospital, Home Health Agency, Skilled Nursing Facility, Ambulatory Surgery facility, Behavioral Health Facility or other facility types as outlined in the application form, please fill out the Facility application and our credentialing team will review your inquiry and respond.

NOTE: FCH does not credential clinics. Individual providers must be credentialed.

After successfully completing the credentialing process (typical turnaround between 60-90 days)

• New providers not joining an existing provider contract will receive a contract to sign via Docusign and a welcome letter with your effective date.
• New providers joining an existing contract will receive a welcome letter with your effective date.

First Choice Health Employee Assistance Program (EAP) only credentials fully licensed masters or doctoral level behavioral health providers who do not require supervision. We are actively recruiting in the following states:

Alaska, California, Idaho, Montana, Nebraska, Oregon, Utah and Washington.

1. FCH EAP does not credential treatment facilities
2. Must have at least a master's level degree in a counseling related field
3. Must be a fully licensed behavioral health provider
4. Must have Malpractice Liability Insurance Coverage - minimum of $1,000,000 per claim and $3,000,000 aggregate.
5. Providers seeking to apply with First Choice Health Network are encouraged to apply to the EAP Provider Network.
