Completion of the Credentialing process is currently taking on average 90-100 days. Please allow sufficient time for our vendor to process these files before reaching out for status. You will receive a welcome letter when your application has been approved. If additional information is needed during the processing of your application, our Credentialing vendor Medversant will make outreach efforts to obtain the necessary information.

Washington State practice location:

1. Complete your ProviderSource application on OneHealthPort
2. Once the application has been completed, select ‘First Choice Health’ under the Health Plans section of the application and attest so that we may access the application and begin the credentialing process.

Alaska, Oregon, Idaho or Big Sky Region (MT, WY, ND, SD, NE, CO, UT) practice location, check here to verify if we are accepting applications for your provider specialty.

• We utilize the Council for Affordable Quality Healthcare, INC. (CAQH) application. Please fill out this form and provide us with your CAQH provider ID number. We will begin the Credentialing process once we receive your inquiry.

OR

• Complete the ProviderSource application on OneHealthPort. You must select ‘First Choice Health’ under the Health Plans section of the application and attest so that we may access the application and begin the credentialing process.

All other states: If you are not practicing in one of the above mentioned states, we encourage you to check back periodically to see if we have expanded our core states.

NOTE: If you currently have multiple active practice locations in more than one state including Washington, you must complete a ProviderSource Application.

If you are a DME, Lab or a facility, please fill out this form and someone will review your inquiry and respond. Please note, FCH does not credential clinics. Individual providers must be credentialed.

After successfully completing the credentialing process (typical turnaround between 30-45 days), you will receive a contract to sign via Docusign (New provider not joining an existing contract) and a welcome letter with your effective date once the process has been completed. New providers joining an existing contract will receive the welcome letter with your effective date.
